Job Description

Major Duties:

Provide support to ensure the integrity of client records

  • Assist in the review and general audit of newly created client records to ensure required elements of the records are in place and communicate issues to appropriate parties as needed.
    • Access client medical records to pull information necessary to assist in responding to request for records or subpoena’s, etc. as directed by supervision.
    • Scans health information into records timely
    • Reviews and Identifies errors and ensure proper correction of entries of scanned files or EHR entries where applicable.
    • Assists teammates as needed to ensure timely entry of documentation into the records.
    • Assist in reviewing and compiling data necessary for purposes of compliance with Licensure, Payer or other regulatory boards auditing request.
    • Assist in providing consultation for staff on specific initial workflows necessary for data to be present in the client’s records as required by agency policy.
    • Assist providers with making request for records from outside providers/organizations necessary to be included with treatment records for the purposes of coordination of care.

Other Duties

Assists when need with education and consultation for staff/clients related to appropriate documentation necessary for release or access to protected health information.

Helps to support the overall Health Information Team on special projects related to record reviews, corrections, or other day to day task that may be needed.

Expands own knowledge of job-related areas by such means as academic study, independent reading and attending outside conferences, workshops, etc.

  • Reviews and ensure understanding of agency policy and procedures.
  • Responsible for other duties as assigned, consistent with established operational policies and procedures.


The employee must meet the following minimum requirements through a combination of education and experience to be successful in performing the duties of this position:


  • High school graduate with basic HIM pre-professional certification desired
    • Any combination of education and experience that provides required knowledge, skills and abilities.
    • Preference will be given to candidates with documentation experience working in an electronic records system as well as familiarity HIPAA, DMAS and DBHDS Licensing requirements.


  • Federal and State laws associated HTPAA and 42 CFR part 2 compliance standards
    • DBHDS Licensing requirements for an array of Mental Health, Substance Abuse, and Developmental Disabilities services
    • General knowledge of Electronic Medical Records systems


  • Able to effectively organize time and manage multiple task
  • Detail focused and able to extensively research issues to determine appropriate action that may be needed
  • Sound judgment and critical thinking skills
  • Interpersonal relations skills, especially in working with a diversity of staff, skills in written and oral communications, administrative. Professional telephone and electronic mail communication.
  • Performing independent work in providing required services and support to management in meeting various information services requirements.
